General Policies

Cancellations

There is a 48 business hour cancellation policy. If an appointment is not canceled 48 business hours prior to the appointment, you will be charged for 50% of the cost of the appointment. Cancellations must be made via phone (925-639-8870) or email (diana.partovi@protonmail.com). In exceptional circumstances, this fee may be waived.

Video Recording

Unless given written permission by Dr. Partovi, clients agree not to video, photograph, or audio record any portion of the appointment. Test integrity is of the utmost importance in neuropsychological evaluations and unlawful recording, copy or distribution of test materials may result in legal action.

Absences

I will make every attempt to inform you in advance of planned and/or extended absences, and provide you with contact information for another neuropsychologist if I am unable to complete your assessment.

Billing and Payments

You are responsible for paying the full session fee at the time of your appointment or at the conclusion of your assessment (as agreed at our initial session). If you are seen exclusively by video telehealth then payment will only be accepted by credit card. Credit card payments are made through an online client portal I will provide to you. 

Contacting Me

Telephone Communications. I am often not immediately available by telephone. I do not answer my phone when I am with clients or otherwise unavailable. At these times, you may leave a message on my voicemail and your call will be returned as soon as possible. I will make every effort to return your call within 48-72 business hours. 

Email and Text Communications

You may email general inquiries and client-specific information (including health data) to my HIPPA compliant email at diana.partovi@protonmail.com. Text messaging is not a secure or confidential method of communication and can be intercepted by unauthorized persons. I will not discuss clinical information by text and prefer that you do not use text except to schedule or cancel an appointment. Should you decide to communicate confidential information by text, you assume the risk. 

Emergency Procedures.

If you are experiencing a life-threatening emergency and need to talk to someone immediately, call 911, the 24/7 Suicide Prevention Lifeline at 1-800-273-TALK (8255), the police, or your local hospital emergency room and ask for the on-call mental health clinician.

Good Faith Estimate.

As of January 2022, you have a right to be provided a good faith estimate for the cost of service prior to engaging in services. Click here to see the attached document explaining your rights and my responsibilities.
